If any changes need to be made to your appointment, please notify me at least 12 hours in advance in order to avoid additional charges:
Any cancellations or reschedules made within 12 hours of your appointment time will incur a 50% missed appointment fee. Any no call, no show appointments will incur a 100% missed appointment fee.
